This really depends on the base media (Michael Bay movies, G1 cartoon, TFP, Earthspark, TFA, Skybound, etc.), but he was originally crafted for the Bayverse.

Red hair, mullet. Below shoulder length in the back. Multiple piercings (septum, labret, eyebrow, navel, nipples, various ear) and tattoos (most notable is the Orion's Belt constellation and "Til all are one" in Ancient Autobot. May sometimes also have it written in Decepticon.) Freckles and scars from dermatillomania & small burns.

In Bayverse: Ends up building a rough leg brace (think Mad Max); one part attached at the knee beneath the pants and the rest locks on from there with a bar under the shoes/feet.
